Program of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility. PACE in MA (MassPACE) is a unique integrated care model for adults 55 years and older available in most areas of MA. This introductory module is for PACE staff and ADRC staff who work with families to help navigate care for older adults.

This module is for PACE staff, professionals working directly with families, and those working in housing entities to help increase creative ways to support the health and housing needs of older adults.
The PACE Interdisciplinary Team (IDT) is made up of healthcare and social service professionals, including doctors, nurses, advanced practitioners, occupational therapists, social workers, and various other service providers. Together, they offer a wide range of medical, social, and wellness services that enable participants to safely live in their own homes.
In this training, you will meet members of the interdisciplinary team (IDT) and discover the unique benefits they provide. You will hear firsthand accounts from team members about their roles and understand the significant impact they have on the lives of PACE participants. Additionally, you will learn how they collaborate to deliver and coordinate services to ensure that participants receive individualized and comprehensive care.
